CareMetx Fax Service Portal — Internal Use & Acceptable Use Notice

This application is an internal CareMetx tool provided to authorized workforce members for administering and monitoring fax services. It is not intended for, and is not made available to, external users or the general public.

This notice is not a separate agreement. Your access to and use of this portal is governed by the CareMetx corporate policies you already acknowledge as a condition of employment and reaffirm through annual compliance training, including but not limited to:

• CareMetx HIPAA Privacy & Security Policies and Procedures
• CareMetx Data Handling, Classification, and Confidentiality Policies
• CareMetx Information Security and Workforce Conduct Policies

By logging in, you affirm that you are an authorized CareMetx workforce member and that your use of this portal — including any access to, viewing of, or handling of Protected Health Information (PHI) — is subject to those policies and to the training you complete at the company level. This portal does not modify, replace, or expand those obligations; it is an operational extension of them.

Access to Fax Data

This portal is a management and monitoring tool. It is not a system of record for Protected Health Information (PHI) and is not intended to store fax document content. However, it provides access to fax data maintained by third-party fax vendors — including the ability to view, retrieve, or link to fax records and images that may contain PHI. When you access such data through this portal, that access is subject to HIPAA and to CareMetx privacy and security policy, and is limited to the minimum necessary for your job function.

Personal Data

Limited personal data of authorized users (identity from the corporate directory, access permissions, and activity/audit logs) is processed solely to operate, secure, and audit this portal. This processing is carried out by CareMetx as part of its normal business operations and is governed by the CareMetx Employee Privacy Notice and applicable data protection law. It is not based on your consent, and no user personal data is sold or used for any purpose beyond operating and securing this system.

Monitoring

Access and activity within this portal are logged and may be monitored for security, compliance, and audit purposes.
